Please verify your coverage with your insurance company to ensure full or partial coverage. Neurological testing may not be covered by your plan. As a result, these tests may be considered to be an out of pocket expense.

If we are out of your network, the full fee will be due upon arrival for service.

Please contact the office to schedule an appointment. Please have your insurance information readily available upon scheduling.
A major credit card is required to schedule and secure an evaluation date. A “No Show” or cancellation within 24 hours will be charged a $100 cancellation fee.
